Pay: $18.25 per hour
About the Role
The Personal Care Coordinator I provides ongoing support to patients with chronic health conditions while working closely with each patient’s care team. Through scheduled outbound calls, this role helps address existing and emerging healthcare needs, connects patients with appropriate resources, and supports improvements in health, well-being, and long-term quality of life.
This is a full-time, fully remote position with benefits. All necessary equipment is provided.
What You’ll Do
- Conduct monthly care management conversations with assigned patients through outbound phone calls.
- Address patient healthcare needs and provide ongoing support between office visits.
- Communicate available resources and services throughout the continuum of care.
- Identify patient-specific concerns, goals, and interventions based on individual needs.
- Develop action-oriented, time-specific goals that support each patient’s care plan.
- Recognize changes in a patient’s circumstances that may require updates to care plans, goals, or preferences.
- Identify barriers that may affect access to care, services, or treatment.
- Provide appropriate health education to patients.
- Escalate patient concerns to the triage nurse team when necessary.
- Maintain accurate, compliant patient records and documentation.
- Document medical history, medications, immunizations, allergies, surgical history, and family history.
- Follow established clinical protocols and procedures.
- Collaborate with coworkers and contribute to overall team success.
Performance Expectations
- Maintain appropriate utilization levels.
- Meet scheduled call percentage expectations.
- Maintain required quality scores.
- Work assigned hours and maintain reliable attendance.
Qualifications
- Current, active compact LPN/LVN license.
- LPN/LVN degree from an accredited nursing program.
- Ability to follow established clinical protocols and procedures accurately.
- Flexibility and the ability to transition effectively between tasks.
- Strong commitment to patient advocacy and high-quality care.
- Ability to build ongoing, collaborative relationships with patients and care teams.
- Willingness to assist coworkers and contribute to team success.
Work Environment
- This position is fully remote.
- All required equipment is provided.
- The role operates in a professional home-office environment.
- Standard office equipment, including computers and phones, is used regularly.
